Access to Records
In accordance with the Data Protection Act 2018, GDPR 2018 and the Access to Health Records Act, patients may request to see their medical records. To request information from your medical record please contact reception.
How To See Your Health Records
- You have the right to see or have a copy of your health records.
- If you want to see or get a copy or your health records, you should complete a request form which requires 2 forms of ID to be verified at the practice.
- You do not need to give a reason for wanting to see your records.
- If you are not happy with anything written in your records you should speak to a member of staff providing your care.
- You will be aware that the practice accepts no responsibility for the confidentiality of this copy once it leaves the practice.
- You will be aware that it is your responsibility to remove any information contained within this record that you do not wish to share prior to passing any contents of the records to a third party.
- You will accept responsibility for this record and for keeping its content secure.
- You will be aware that this is the only copy of this record that will be provided. Should you require a further copy of this record for any other third party it is your responsibility to copy it for them (excluding any copies of records issued after the copy date) it will be chargeable.
