Community Links Practitioner

Gerry Mitchell is our Community Links Practitioner and you may be referred to him by your GP or another member of the practice team.

The CLP helps people access community resources to assist them to live well.

Gerry can help you with any non-clinical issues you may be experiencing and he can signpost you to services in our area that you might benefit from. For example benefits or housing support, carer or volunteering opportunities or access to activities such as computer literacy classes, training and employability support. He can also access fun activities such as walking and cycling or cooking to improve your health.

You can make an appointment with Gerry at reception without a referral from your GP.

Bringing the Community to You

Every Wednesday from 09:30 – 11:30 we have a local organisation providing drop-in advice and support for you to access in our waiting area. Organisations offering advice and one to one support for a variety of different needs including: benefits, housing, training, employment, support with physical and mental health or addiction. We also have organisations offering support to carers, families in need, relationship problems or for people with long term conditions such as dementia, stroke, MS, COPD or cancer.

There are also various classes that patients can attend some of which are listed below. Please contact Gerry for further information on these.

  • Yoga for Pain – Tuesday afternoons
  • Tai Chi – Wednesday mornings
  • Arts and Crafts – Wednesday mornings
  • Walking Group – Wednesday mornings
